Patient Registration

The doctors welcome new patients who live within our practice catchment area.

If you are currently registered with another practice within the Redditch area we advise you remain with your current practice until such a time that you are asked to find an alternative practice for reasons such as moving out of your practice catchment area. 

As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire. 

Medical treatment is available from the date of registration. Please contact reception for further information.

You may print off a registration pack, complete it and bring it in with two forms of ID details of accepted documents are noted in the pack but must  include one form of photo identification and one of proof of address dated within the last three months.

For children aged 5 years of age and under we ask you to present your child's red book health record at the time of registering and please note we cannot register a new baby without their NHS number. 






Sharing Your Summary Care Record (SCR)

The NHS Summary Care Record (SCR) is an electronic summary of key clinical information (including medicines, allergies and adverse reactions) about a patient, sourced from the GP record. It is used by authorised healthcare professionals, with the patient's consent, to support their care and treatment.If you Havent already consented to opt in to share your SCR please download and complete our consent form and return to surgery reception. If you wish to Opt out of the shared SCR please enquire at reception and one of our team can provide the opting out application. 




Patient Representative 

For anyone over 16 years of age that requires another adult to represent them or disscuss there medical matters including managment of prescriptions, correspondance with the surgery or to book appointments on there behalf in accordance with information governance and the GDPR we require consent from the patient. A patient consent form can be found below this can be downloaded and completed and returned to surgery.

when returning the consent form the patient must be in attendance and a member of surgery staff must witness the patient signing the form.

If the patient lacks the capacity to sign or is unable to attend surgery then the patients representative must attend with both their own and the patients photo identification in order for the surgery to confirm implied consent.

Temporary Registration


If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for up to 15 days. This will allow you to be seen at local practice and still remain a patient of your permanent GP.


After 15 days your your temporary residency will expire and we will return any records we hold to your regular GP. If you still require treatment from us you will need to register permanent/regular patient.


To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Please be aware that due to both legal and contractual requirements all temporary residents must present to the practice with photo identification. if you don't not have any photo identification we suggest you attend an alternative service sure as out of hours GP, minor injuries department of emergency department
